1. Prepare necessary documents

In case the original file is lost

The original document is lost but the class C license is still valid:

• Copy of CCCD/ID card, valid passport.
• Copy of Class C license (if any).
• Declaration of cause of loss of original records.
• Application for re-issuance of class C license.
• Health certificate from district-level hospital or higher.
• 3 3x4 color photos.

The original document is lost but the class C license has expired:

• Copy of CCCD/ID card, valid passport.
• Original C class driver license (if any).
• Declaration of the reason for the loss of the driver's license record.
• Health certificate issued by district hospital or higher.
• An application for re-issuance of a Class C license.
• 3 3x4 color photos.

When the original file is available

• Copy of ID card, CCCD, valid passport.
• Original dossier of license for class C cars.
• Application for re-issuance of a class C driver's license.
• Health certificate from district-level hospital or higher.

2. Submit application

Dossier of application for re-issuance of a lost or damaged class C driver's license shall be submitted at the receiving and returning department of the Department of Transport or the Directorate for Roads of Vietnam.

3. Fees

Circular 188/2016 of the Ministry of Finance stipulates that the fee for re-issuance of a class C driver's license is VND 135.000/time. In the case of re-examination:

• Re-examination of the theory test: 90.000 VND/time.
• Re-examination of the practice test in the picture: 300.000 VND/time.
• Re-taking the practice test on public transport: 60.000 VND/time.

4. Get a re-issued driver's license

After 2 months from the time of submission of all the required documents and fees, if the driver's license information is in the records of the testing management agency and is not handled and seized by the competent authorities, then C-class driver's license will be reissued.
